Send in your 6 till receipts or 5 receipts with a coupon (only 1 coupon per application from either a promotional press or magazine advert or downloaded from our website www.tabletable.co.uk/waitingforyou) attached to either a promotional leaflet or enclosed within a promotional collector wallet by the 28.11.08. Fill in details requested on wallet/promotional leaflet including your name, address, postcode, e-mail address, mobile phone no. & age range enclosed with a stamped addressed envelope to; Table Table Offer, The Leapfrog Group, Nelson House, Park Road, Timperley, Cheshire WA14 5BZ.
- Offer entitles customers, who correctly redeem the offer, to 3 vouchers - a Pamper voucher, a Pantomime Voucher and a Citybreak voucher. Pamper voucher entitles the holder to a free Pamper treat at a participating venue or with a participating practitioner. Pantomime voucher entitles one child to free admission to a pantomime/ show when one full priced adult admission ticket is purchased (some venues exclude OAP's as a full paying adult - please check when booking). Both Pantomime/ Pamper offers also feature alternative offers from that stated - these will be specified. Citybreak voucher entitles one person to a return flight (excluding tax which is payable when booking is confirmed) and 2 nights in a 2 star hotel single room accommodation only to one of the following destinations (Paris, Budapest, Madrid, Amsterdam, Prague & Barcelona). Flight departures only available from Manchester/Liverpool and Gatwick, Stansted and Luton airports. Other regional departures available at a supplement. Customers will need to state 3 alternative destinations and 3 different months of travel on their application form. Citybreak application forms need to be received by 13.02.09 and final date for travel is 19.06.09. Citybreaks booked through Concepts for Travel (ATOL 91265344, ATOL 9317). 2nd passenger supplement available from £199 (excluding tax) in a twin/ double room. Minimum 45 days advance notice required before travel. Flights and accommodation subject to promotional availability.
- All 3 vouchers exclude use/ travel during Christmas, New Year, Bank Holiday periods, Easter and special event days such as Valentines Day. Full terms for the respective offers can be found on the vouchers.
- Only one claim per person, per household. No multiple or third party claims accepted.
- Please allow 28 days from receipt of your claim for voucher delivery.
- There is no cash alternative to this offer.

- The promoter and administrator accept no responsibility for claims that are lost, delayed or damaged in the post. Proof of posting is not accepted as proof of receipt.

Pantomime offer
- The voucher is valid until 19.06.09. Any additional day/date/time restrictions are clearly detailed within the theatre listings. Please check prior to your visit. However many Pantomime venues will stop showing Christmas pantomimes within the first two months of 2009.
- This offer entitles one child to one free admission to a pantomime/ show when one full priced adult ticket is purchased. Any differences to this offer are clearly detailed in the listing. Only one voucher per visit, per booking can be used. The offer may exclude OAP's/ adult concessions, please check when booking.
- Entry to a participating theatre/venue is subject to the terms and conditions of that particular theatre. These are available on request from each theatre direct.
- Each individual theatre/venue retains the right to vary prices and availability. Contact theatres/venues direct for details of opening times, show times, availability and prices.
- This offer is subject to promotional availability.
- You are responsible for making your booking. Please state that you hold a 'Table Table kids go free pantomime voucher' when making your booking.
- The voucher must be handed in when you arrive at your chosen theatre/venue.
- The promoter can give no guarantee regarding availability at featured theatres/venues at any time.
- Each theatre has confirmed its willingness to participate in this promotion. The promoter cannot be held responsible for any theatre declining acceptance of a voucher for reasons of their own, nor can they be held responsible should any participating theatre/venue cease to trade. An alternative theatre/venue can be found in the theatre/venue listing.
- The promoter shall not be held responsible for the suitability or otherwise of a participating theatre/venue. Any dispute must be settled direct with the participating theatre/venue concerned.
- The promoter shall accept no liability for personal loss or injury at a participating theatre, as far as permitted by law.
- Your voucher may be used once only. Only original vouchers will be accepted.
- The voucher cannot be used in conjunction with any other promotion or offer.
- The voucher cannot be exchanged for cash or used in any other way than that described
Citybreak offer
- This offer entitles 1 person to a city break in one of the following destinations: Paris, Budapest, Madrid, Amsterdam, Prague & Barcelona. This offer excludes airport tax which must be paid upon confirmation. The offer includes a return economy flight with selected airlines to one of the listed destinations and two nights' single room accommodation in a two star hotel for 1 person. Room/hotel upgrades are available on request. All flights and accommodation for the duration of your stay must be booked through Concepts for Travel. A 2nd passenger supplement is available from £199 excluding airport tax in a twin/ double room (upgrade from single room).
- Flight departures are available from London Airports (Gatwick, Heathrow, Luton and Stansted) or Manchester/ Liverpool. However, under the terms of this offer, Concepts for Travel may not offer flights to all the destinations from all the listed departure airports. Concepts for Travel also may not offer all flights direct from all departure airports to the listed destinations and non-direct flights may incur a supplement and/or a short stopover. The departure airport and routes available will depend on the destination, time of travel and airline used. The airline and hotel used will be at the discretion of Concepts for Travel. Other UK departure airports may be available at a supplement. All flights are subject to promotional availability.

- Airport taxes, supplements and insurance (if required) are excluded and, where applicable, are payable on confirmation. Airport taxes vary and are dependent on destination, time of travel and airline used. This offer does not include transfers, meals, drinks or any other extras. All extras must be settled with the hotel direct. The promoter and Concepts for Travel cannot be held responsible for any disagreements that may arise concerning the amount of the account as presented by the hotelier. Any dispute must be settled direct with the hotel in question.
- Only one application may be used per household and per booking and only one application may be enclosed per envelope.
- The voucher holder must give three alternative destinations and three different months of travel on their flight application form. Applications not completed in this way will not be processed.
- To cover credit card commissions and other expenses payable by Concepts for Travel, all credit card payments are subject to a 2% charge.
- It is strongly recommended that adequate travel insurance is purchased to cover all passengers on your booking. Concepts for Travel will be pleased to assist you with travel insurance requirements but this is not compulsory. However, Concepts for Travel cannot be held responsible if you fail to take out adequate travel insurance. By confirming a booking with Concepts for Travel you are agreeing, on behalf of all passengers, to this contract. You will be asked to confirm that you have read and accept this on your application form. Concepts for Travel will be unable to process your application should you fail to do so.
- There is no cash alternative to this offer.
- No cancellations or amendments by you are possible after confirmation of your booking. No refund of any monies paid will be possible.
- The promoter and Concepts for Travel cannot be held responsible for any alterations or cancellations to flight schedules, routes and destinations and reserve the right to use alternative suppliers. We reserve the right to withdraw this offer without notice in the event of war, earthquake, catastrophe or similar event.
- A minimum of 45 days advance notice before travel is required. The lead name on the booking must be aged at least 18. A maximum length of stay of 30 days applies.
- The usual terms and conditions of special fare IATA tickets apply to any flight booking. A copy of these may be requested from Concepts for Travel
- Your tickets and/or documentation cannot be resold or given away.
- The applicant will be contacted with an offer of a return flight and accommodation which matches the requests on the application form. Once a city break offer has been made, you must confirm acceptance immediately. Flights or accommodation cannot be held on your behalf.
- This offer may not be used in conjunction with any other voucher or promotion. Proof of posting is not accepted as proof of receipt.
- No responsibility can be accepted for lost, delayed or damaged application forms. Please be aware that it is not the responsibility of the promoter or Concepts for Travel to replace the application form if it is lost.
- It is the voucher holder's responsibility to arrange Passport and Visa requirements. All passengers travelling must ensure they have at least six months validity on their passport on the return date back to the UK.
- This offer is valid for travel until 19.06.09 All travel must be completed by this date. Completed Citybreak application forms must be received by 13.02.09.
- This offer does not represent an entitlement to travel on a specific route on a specific date or to stay at a specific destination on a specific date. There can be no guarantee that you will be offered your preferred choice of destination and/or date.
- This offer is not valid for travel during Christmas, New Year, Easter or Bank Holiday periods. Other date exclusions may apply depending on the destination and time of travel. Concepts for Travel will advise you of this at the time of booking.
- The city break offer is subject to promotional availability. Availability can be affected by a number of reasons beyond the control of the promoter or Concepts for Travel. These include adverse weather conditions, fuel increases, terrorist activities, political incidences, supplier change of ownership, supplier insolvency and airline operational difficulties.
- The voucher is issued by Concepts for Travel (IATA 91265344, ATOL 9317).
- Please be aware that the airline companies may be running their own unrelated reduced flight offers at the time you are applying with your voucher. It is always worth checking to see whether any of these offers are available before you use your Citybreak voucher and whether you could make better savings.
- We will be able to investigate complaints or queries up to 54 days after the travel completion date 19.06.09. Once the 54 days have passed, we will not be able to provide any further investigations and the offer is deemed closed.
- All travel detail initially provided cannot be changed once the application has been submitted.
- Most low cost and charter flights do have a charge for baggage which is not included in this offer. You can either choose to pay at time of booking or at the airport. If you choose to pay at the airport we strongly advise that you check exact baggage charges direct with airline a few days before departure and allow extra time for checking in.

Pamper offer
- This offer is valid for use until 19.06.09 excluding Christmas, New Year, Easter, Bank Holiday periods and special event days. Any additional date restrictions are clearly detailed within the venue listing. Whilst most venues/specialists participate in this promotion year on year, some may change. To avoid possible disappointment, please check with the venue/specialist direct. 2. This offer entitles the holder to one of the following Pamper Treat offers:

- To book with a Nutrition Therapist or Life Coach you must call 0845 050 2442 (Monday to Friday 9am - 5.30pm, calls cost no more than 5p per minute for BT customers, other providers may vary). You will then be contacted within 48 hours to arrange a time for your session; you will be given the phone number of your allocated coach and the address to send your voucher to. You must then post your voucher to arrive before your allocated session time. Then simply call your coach/therapist at the agreed appointment time for your session. If either the coach/therapist has not received your voucher, or you do not call at the allocated session time then you will lose the offer.
- Some activities may require a certain level of fitness or ability. If you are suffering from any medical conditions or injury, consult your doctor before participating. You may be required to complete a health form.
